Hello to all,

 

Our 4.5 year old Irish Wolfhound, Fiona, is two weeks post diagnosis with osteocarcoma.  The tumor on the distal radius was discovered after she experience sudden, painful lameness.  Our veterinarian thought she had injured her elbow, and the tumor was caught on that X-ray.  Chest xrays show no mets, but we understand that micrometastases are quite common.  We have had a consultation with oncology at the University of Minnesota and are on track for a complete amputation of her left front leg and chemotherapy.  She may be eligible for a study, the start of which is a biopsy and CT scan, with surgery scheduled 10 days after.  At this time, her pain is very well controlled with Metacam and a relatively low dose of Tramadol.  She is as active as she ever has been, often walks without any discernible limp at all, and wants to be outside (unsupervised, please, mom) and play with her German shepherd sister.  My question:  Should we proceed with amputation immediately or should we wait until we have exhausted all medical pain relief options?  The tumor is quite small, showing nothing on the outside of her leg, not overtly painful on exam, and all vets have said they believe it to be very early -- leading us to believe the danger of pathologic fracture is reduced.  Part of our debate is that we live in Minnesota, where we currently have ice and snow, which will be a slippery surface for a new tripawd.  We are likely to have the chance of these conditions for a few months to come, but as the winter wears on, if we do have slippery conditions, they tend not to last as long as the weather trends to warming.  So, immediate amputation?  What are the benefits and effects on prognosis and survival time?  Later amputation?  Same questions.  We would love to hear your experienced opinions!  Thanks in advance.

First, welcome. Your future forums posts will not require moderation. Finnegean the three legged Irish Wolfhound was one of te first Tripawds members Jerry met in his travels, and Darcy Deerhound was quite a hero around her so we have a soft spot for the big dogs...

You bring up a good point. There is some discussion in the veterinary community about how removal of the primary tumor can expedite spread of metastatic cancer. Once the main tumor is gone, some believe the cancer is more likely to spread. Just a thought, please keep in mind that we are not vets! Consult with your oncologist on the matter and consider reviewing our veterinary oncology video interviews.

Our 7 almost 8 year old Alaskan Malamute presented as Fiona,  lameness.  Tiffany, was a little lame at first but the "injury"  became increasingly more painful.  This is because the cancer destroys the bone from the inside out.   If it was diagnosed as OSA then the pain will only get worse.   It sounds like the specialists are suggesting exactly what we were told.. amputation and chemo.

 

To wait or not to wait.. 10 days might be fine but when Tiffany presented lame it got worse within about 10 days to the point she was in obvious pain and wanted not to be around us anymore..in other words it went down hill quickly.  Luckily we were able to schedule her surgery quickly.. If I could revisit what we did, on first presentation on lameness I would have insisted on an xray..(our vet suggested giving her rimadyl for two weeks and see how she did)  I wish now we did not loose those two weeks (and the next week then discovering the cancer in an xray) so really three weeks.

 

After the tough two weeks following amputation ( sleepless nights) she went on her first walk Sunday and what a transformation.. She is now happy, bright eyes and back to "talking" not whinny like most northern breeds do.

 

I wish you luck in your decision.. nothing seems to be easy.

Sitting with the sweet Fiona into the wee hours.  Her amputation is tomorrow and we must have her at the University by 7:00 a.m.  All assurances from professionals are that we are doing what we should and it's the right thing, but I still have trepidation.  Thank heavens she does not.  Wish us well, tripawd compatriots!

Jill is a 9-year-old tuxedo kitty. She was diagnosed with Osteosarcoma in June 2012 on her toe in her right hind leg. Having doubts is normal, this is a big decision and a life-changing one. But rest assured, you are doing the right thing to avoid any horrible fractures or more severe pain from the osteo, and Fiona will AMAZE you. The recovery will have some ups and downs but overall you'll see that it's a small price to pay for more quality time together. We will be here to help every step of the way.

Sending good thoughts for a speedy recovery.  I know how you feel, I don't think I slept the night before Trouble's surgery either.  Probalby part of the reason I cried all morning.  It is a really hard decision to make. Once you see an adjusted, pain free Fiona, you will know you made the necessary decision.

Abby: Aug 1, 2009 – Jan 10, 2012. Our beautiful rescue pup lived LARGE with osteosarcoma for 15 months – half her way-too-short life. Here's the news:  I have not yet seen Fiona post surgery, but according to the surgeon, she sailed through the surgery (yesterday at 8:00 am), and through recovery, ate a full meal eagerly at 9:00 pm, sat up and stood up on her own, would not ride the cart to go outside, but insisted on walking, and peed!  I am still full of trepidation about her return home, which will probably be late this afternoon, but so incredibly relieved to have these good reports so far.  Thank you to all for your support -- what a great community.
