Cyclophosphamide (=cytoxan) in metronomics: who's used it and what did you think?

At Jackson's next chemo, his third, his onco suggests we could start a metronomic protocol, including Cyclophosphamide (= cytoxan).  I am still fuzzy on the details, whether we are to give it every day, every other day, and if we are to continue use of carprofen (= rimadyl).  I believe the dosage is 10mg per day - Jackson is 58lbs.  So....what does this cost folks?  I've emailed Diamondback Drugs for an estimate, for both Cyclophosphamide and Carprofen.  I read that some of these metronomics can lead to bladder infections, but that they are less common in boy dogs.

If you used these, what did you think?  Did your pups tolerate Cyclophosphamide well?  How long were you on it, or still on it?  Side effects?

I was taken aback that she's wanting to start metronomics long before he's done with IV chemo or even before signs of lung mets.  I think she's wanting to be as aggressive as possible.  I like that, but I also fear we're pumping Jackson full of drugs and that's worrisome.

Amputation 12/28/12.  Chemo (carboplatin) started Jan 10, 2013 and ended on April 5, for a total of 5 doses. He handled carbo like a champ!  No side effects.  We started metronomic therapy at his third chemo and have been also doing some holistic treatments.  He's a lively, playful 10 year old huskie-boarder collie and a very proud member of the Winter Warriors!  Our love. Our funny little guy!

While it does seem like a lot of drugs, metronomics is a very low-dose way of keeping a steady dose of chemo in his system without giving him dangerously high levels of it.

We've seen prices for metronomics all over the board. When I was on it, we used generic Metacam & doxycycline, which was relatively cheap, as well as generic cytoxan. All of these drugs we purchased online from various pet pharmacies, but you can also get them at Costco very inexpensively. I want to say it was somewhere around $100 a month but maybe a little more or less because we were buying in amounts that lasted more than 1 month.

Finally I can contribute! lol

 

My dog polly is in both drugs currently and I just re-filled her prescriptions last night!

 

So for the Cyclophosphamide- 25 mgs (which we have to quarter for her size of 15 lbs) a qty of 30 was 67$ without any kind of insurance. If we had bought 100- I was told it would have been around 200$. Since this is the first time doing this with polly, we decided to go with the lower amount since that will already last us 4 months.

 

As for the Carprofen for a 2 month supply for polly it is 58$.

 

We are doing the metronomic protocol and sat down and priced it all out for polly. We are finding the 5 step protocol to cost us 90$ a month for all of the meds and vitamins and we have started her on a high protein diet- which we make and mix into grain free merrick canned dog food and that is running around $30 a month for the canned food and another $25 a month for the free range chickens and beef and carrots etc.

 

The reason we chose to go wirth the metronomic protocol, was that our vet had a ton of luck with it. Dogs with more advance cancer than polly made it a full year this way. Shes treated many and all of them are living longer with this treatment then the others who chose different options.

 

We are also only giving polly triple filtered water, which we also boiling before giving it to her. We live in an area with wells and septic tanks and after reading some of the cancer books we realized that our water could have all kinds of contaminants in it. I never even thought about that.

 

So far (its only been 2 days on this metronomic protocol) Polly is doing great! This morning she even pushed another dog out of the way to get to the water bowl, which she hasnt done since amputation. So we are starting to see that old fiesty girl come back-which is just amazing!

Thanks for such info!  Did you get your drugs from your vet or from another or from an online pharmacy?  I like your prices!  I'm glad to read that she's doing so well on them, so far, and that there are other success dogs in your same practice.  Seeing and hearing about fellow patients, who are doing well, is no nice.  That's great.  You didn't mention she's on antibiotics, like doxy?  No?  Just an NSAID and chemo?  I presume you're wearing gloves when you touch her chemo pill?  Since she's a she, does this mean she's getting her chemo pill every other day?

Jackson is doing great!  I think today may be the day he begins to slow down from his IV chemo.  We'll see though, as it's PuppyBowl day and we've got some of his favorite people coming over.  I guess this means TOMORROW will be his 'crash day'!

I hope Polly has a good day today.  Again, thanks for all your help and advice.  Keep it coming!

Polly is on a 5 step; which includes the cyclo and carprofin, as well as doxy and then 2 different immune system boosters. 1 for bones (since she has osteosarcoma) and 1 for whole body.

 

Everyone has made it very very clear to wear gloves when administering the cyclo. So I got out a face mask and gloves and a pair of scissors from the dollar store and cut them all into quarters last night.

 

She is actually taking it every night, since she has never had any problems with her bladder or anything else for that matter. Before the cancer diagnosis, I swear I thought she was a super dog because she never ever got sick. In the 4 years I've had her not even a doggie cold.

 

The doxy we order online. The cyclo we got at target. A lot of pharmacies do not carry it, expecially in the low milligram amounts because a human taking that medicine, would need a much higher level. So before you fill it, call around and see who has it. Like I said, my target had it, but not CVS, rite aide, giant or a local phamacy. So it was a little frustrating. Most can order it though and since you have time, you can always have them do that.

 

The carprofen- we got though the vet.

 

I dont think the prices are hateful. We didnt do IV chemo because we just couldnt afford it after the cost of the amputation. The specalist said the 4-6 treatments would run between $500-700, each...

I like this plan since my vet has had so much luck on it, and studies show it to be very effective. Plus giving it to her on a daily basis makes me feel like I am really doing something to help her kick cancers butt!

Just wanted to chime in and say that my dog, Spirit has been on Cyclophosphamide for the past year. He's had zero side effects. He is about 47 lbs. and takes a small dosage daily (8.1mg). We go in for regular check ups every 6-8 weeks and blood work has always come back good. The first and only issue we've had was during his last checkup where his white blood cell count came back a bit low so his oncologist is giving him a break from it.

As for cost, we get Piroxicam and Cyclophosphamide from Diamondback Drugs for about $125/1.5 month supply.

We did Cytoxan and piroxicam every day w/ Abby once we stopped her IV chemo. Interesting that they now do them together. Abby was on it for a long time - probably more than 9 months and never had any problems with it.

I can't remember the cost, but it was much cheaper than a lot of the other stuff we did!

Because I am still new to all of this, I do not really have any additional thoughts on this discussion, but I just wanted to say that since we are probably going the metronomic chemotherapy route with our Bailey girl, I am finding it really helpful to read about everyone's discussion on this topic.  So, thanks.  (I do not know yet what particular drugs our vet wants to use for this treatment.)

The CVS down the road from me only carries Cyclophosphamide at 25mg or 50mg size.  That's not helpful.  We need 10mg.  Diamondback drugs carries it.  Here's their quote (shipping to CT is ~$6): We have the cyclophosphamide 10mg #30 = $31.50, and the Carprofen 50mg #30 = $27.95.

That's great!  As Jackson's onco was estimating $130 for 30, 10mg Cyclophosphamide.

I'm also going to check with a few other human pharmacies in the area and Jackson's regular vet.

I've had great luck with Roadrunner Pharmacy.  I have pugs and lots of meds don't come in small enough doses for them.  I have found RR to be very helpful and efficient, in fact I just spoke to them today.  I am getting liquid gabapentin from them for Tani and we are trying new flavors (no xylitol!).
