Cyclophosphamide (=cytoxan) in metronomics: who's used it and what did you think?

Thanks for posting all the pricing tips on these meds, you have all helped me out too.  Bailey is going to start metronomics tomorrow and the vet is prescribing cytoxan and meloxicam. He did not say anything about an antibiotic for her, but I noticed that in the info that I have looked up about metronomic chemotherapy and also here in many posts about the topic, many of you have mentioned an antibiotic in addition to the other two drugs, particularly doxycycline.  I will ask my vet about this tomorrow, but has anyone else had a vet do the metronomic chemotherapy without the antibiotic?

Many vets are moving away from the antibiotic. It's now being thought that it's not as anti-angiogenic as researchers used to think, so some are skipping it, however others are still using it. The choice all depends on the onco and what they've found success with based on their client histories. You should definitely ask about it but your onco can go either way.

Jackson's "progressive" onco is not prescribing an antibiotic.  He'll be on an NSAID (carprofen = Rimadyl) and cyclophosphamide (= Cytoxan).  I'm waiting to hear from DiamondBack drugs what it'd cost to have both of his drugs compounded into a single pill (is that even possible for those drugs?) and waiting to hear from RR about the same thing.

ACL tear in right hind leg 12/5/12 and scheduled ACL repair surgery 12/21/12. Pre-op xrays revealed osteosarcoma. Amputation 12/28/12.  Chemo (carboplatin) started Jan 10, 2013 and ended on April 5, for a total of 5 doses. He handled carbo like a champ!  No side effects.  We started metronomic therapy at his third chemo and have been also doing some holistic treatments.  He's a lively, playful 10 year old huskie-boarder collie and a very proud member of the Winter Warriors!  Our love. Our funny little guy!

Update: RR won't give me prices until they have an Rx from his onco.  We're under no obligation to purchase then, so I'll certainly see about their prices.  Free shipping UPS nationwide.  That's good.

I just spoke with Jackson's onco about his metronomics : he'll be on Cyclophosphamide (10mg) and Carprofen (50mg) everyday, and given in the morning as Cyclophosphamide can lead to bladder problems.  In addition, she likes her patients to be on MSM during metronomics .  He already gets 200mg in his joint supplement, but for his size she'd like him to be on ~400-500mg.  That's easy enough to do - go to GNC and buy a bit more MSM until we finish his current joint supplement, then buy one with more MSM.  He can start this cocktail the day of his third chemo (2/21) or wait a week or so; I'm inclined to wait simply to give him time to process the IV chemo and to get quotes, receive drugs from Diamondback or RR.  After about a month of being on the Cyclophosphamide and Carprofen cocktail, we can add in an antibiotic (doxycycline).  She likes to wait to add this in as it's most often the antibiotic that causes stomach upset.  If stomach upset happens, he won't stay on the antibiotic.

The bummer is that this program requires a lot of bloodwork, labs, and visits - but the first few visits will coincide with his IV chemo, so I guess there's 'cost saving' and 'time savings.'  There are complete CBCs and uranalyses done at monthly intervals (plus an onco exam fee, of course), and then they are spaced out to like every 8 weeks, 12 weeks, etc.  We could have his labs done at his regular vet (it's 15min away vs 45min away), but the onco still wants to see him.  So, while the drugs may not be very expensive, monitoring him and labwork will add up.

That regimen sounds comprehensive. We didn't have as much labwork done when I was on it but I wonder if that's because I wasn't also on IV chemo at the same time? Would the labwork they're requiring be necessary if he was only on metronomics ? And will it be required once he's done with IV chemo but still on metronomics ?

Did the onco say what the MSM is for beyond its usual joint supplement purpose?

Let us know what Roadrunner says. It makes sense that they can't quote you yet.

Hello, I did talk to Bailey's vet specialist and he does not use an antibiotic as part of his metronomic protocol.  We decided to go with a small, local family owned pharmacy that our vet reccommended for her prescriptions, and from the pricing I am hearing from others, it seems that they are fairly priced.  Bailey is on Cyclophosphamide (10mg capules) once a day and Meloxicam (1.65mg/mL chicken flavored suspension; she gets 1 mL) once a day.  The Cyclo- was $37.50 for a month supply and the Meloxicam was $45.00 for a month supply.  We started the metronomic chemotherapy on Thursday and so far Bailey has taken the meds without any problems.  In fact, she thinks the chicken flavored stuff is the bomb!

As for rechecks, now that she has started the metronomic protocol...her vet wants to do CBC rechecks every 2 weeks for the first 2 months.  (This is a quick walk-in and out at our vet without appointment. Luckily our vet is nearby.) Then, he wants to do recheck chest radiographs and recheck ultrasound at the 2 week mark, 6 week mark and then every 3 months after that.

Sounds like a great plan (and cost-effective too...bonus!). Keep us posted on how it works. Good luck!

Sounds great!  Excellent prices, too, and a local pharmacy = bonus points!  Could you ask them which pharmacy they get it from (I understand these doses of Cyclophosphamide are always being compounded by compounding-pharmacies, so it's likely your vet uses one of these businesses).  It'd be helpful to start a list or a blog post (Hey, Jerry, here's an idea!) of various, reputable, compounding pharmacies.  Jackson's routine vet (not his onco - I don't know who they use) uses Wedgewood http://www.wedg.....petrx.com/.

As for the MSM, I forget exactly what Jackson's onco said was the purpose, but I believe it is to aid the body metabolize the chemo and cut down on additional risks to his kidneys.  It's simply a bonus that MSM is often found in joint supplements that all us Tripawders love.  At his size, she'd like him to be on 400-500mg MSM/daily.

Libby starts her metronomics tomorrow and it cytoxan and rimadyl.  I asked the vet about the antibiotic and she said you can't dose it high enough to be effective in the blood stream.  I may not have that worded exactly right.

I need to find another pharmacy for the cytoxan because it's $70/month through my vet's office.

Liberty (Libby) was diagnosed with OSA on 1-22-13.  Right front amputation on 1-31-13. No IV Chemo. Metronomic Therapy started 2-19-13 along with supplements and some home cooking. Lungs clear until 1-06-14.  She's still her happy, hoppy, bossy self.  Living the dog life to the fullest and a proud Winter Warrior. :) RIP my Libby 4-21-03 to 3-19-14
