At what point should we start getting concerned. She is going outside and peeing twice a day

She's probably not eating as much as normal, so there's not as much stool inside; plus pain medications can be constipating.  You can give her some pure pumpkin to help.  1-2 spoonsful every day of pure pumpkin, not the kind with all the spices in it - the can should say pure pumpkin.  My dogs will usually just eat it for me.  You can also put some into an ice cube tray to freeze so that it doesn't go bad in the fridge and just thaw out  one cube at a time for her.

She also isn't moving as much as she normally does, walking increases peristalsis, which is the movement inside the bowels.  And maybe not drinking as much water as normal.  It's all a perfect storm.  She will go eventually.

Yep, take it from nurse Donna (she really is!), poopiconhappens! The more liquids and fiber-rich add-ons she takes in, the faster it will happen. As always, if you're worried let your vet know. But for the most part, this is what we see around here, some dogs just take a while to go. I'm thinking it might even happen today for her! Keep us posted.

Thank you both.

She still hasn't pooped and now she hasn't peed since yesterday around 10am, so about 34 hours. She just doesn't have any desire to get off the couch. Her appetite was declining a bit. I spoke with the vet and they advised that we take her off all of her medication (carprofen, gabapentin, amantadine, and codeine) and see if her appetite returns. She did have a bowl of rice and ground beef this evening right when the medicine was starting to wear off and she doesn't appear to be in pain so I am hopeful we will start to see some improvement soon.

Has she peed?  The pooping  not a huge concern, but the peeing is important.

Ok it’s definitely time to let your vet know, the not peeing is a concern. I’m on my phone but look up how to express a dogs bladder here to see if you can get her to go ASAP. Let your vet know as soon as you can and keep us posted.

She has been off meds now for 24 hours now and she seems more active today. She ate breakfast and she peed. Still no poop.

She did whine a little this morning so I worry she might be in pain. If she whines more I met give her carprofen since she was taking that for a week before the other medications and didn't have any side effects

So glad she peed!!!!!    So let your Vet know if she starts to  have issue again with delayed peeing.

Glad she's perkier today but sorry she's showing pain signs.  Talk to your Vet about adding  back a lower dose of Gabapentin.  The carprofen is basically  an anti-inflammatory  and not much help with major surgery pain by itself.

My tripawd Frankie had TPLO surgery awhile back and didn't poop until day seven!   Hopefully she wont wait that long.  And maybe by eating more now she'll poop.

Well that's awesome about the peeing! You can help with the poop issue by adding steamed pumpkin pulp or squash to her food, even olive oil can help too.

It's pretty early to have her off pain medication. When was she whining? Was it after any kind of activity? Could she be getting too much right now? She should still only be doing very controlled leash walks to go potty, until stitches come out. Most dogs tend to be on pain control for at least 10 days, so definitely talk to your vet about her pain signals
