The week started out so great!  Belle was back to her old self, we were so excited and ready to get on with a "normal" week.  Tues we took her to the surgeon for her 2 week check and she passed with flying colors, she even got the green light to go back into the pool, she was so excited.  She swam swimmingly with only 3 legs.  I figure we had this Osteosarcoma licked and the chemo was just gonna be a secondary treatment.

Yesterday was to be her first chemo treatment.  Gordy dropped her off at 9am and they said she would be finished after lunch.  By 2:30 I had not heard from the vet, so I called only to be told she had not even begun her treatment yet.  I asked to speak to her oncologist to find out what was going on.  Dr Coyle said the treatment (carboplatin) had not been administer yet because Belle's creatinine was elevated.  What concerned her and NOW us was her levels were prefect before surgery 0.8 and now they were 2.5 (high normal is 1.8).  They were waiting for Belle to give a urine sample to see if she was dehydrated.  She said she would call back shortly with more info.  An hour later, I called them, Dr Coyle said they had to remove some urine from her bladder with a needle since she refused to pee.  Her specific gravity was low and she decided to hold off on the carboplatin for the day.  We talked about options and have decided to wait one week and re-evaluate her levels next Tues.

Here are my concerns

1. What caused her creatinine level to be so high.  After much research, I believe it was 2 weeks of Rimadyl.  Now my concern is, did Rimadyl damage her kidneys and will her levels decline without taking any more or is the damage permanent?

2.  Dr Coyle stated that the carbo can be given with her levels, but she does not recommend it as it can also do damage to the kidneys.

3. Last night Belle threw up twice, could it be the stress of being at the vet all day or something else.  This morning she will not eat her food, even with yummy chicken broth on it.  She is very lethargic also.

4.  The other chemo treatment they can use is Adrimycin.  Dr Coyle scared me to death when she told me the side effects both during it being administered and afterwards to her heart.  We would have to see a cardiologist before treatment and then during. WOW.

My questions for everyone out there:

1. Has anyone had the same problems.  Has anyone had any adverse reactions to Rimadyl or Tramadol that have affect the kidneys.  And if so, did it reverse over time or not?

2.  Thought on Adrimycin?  Thought on Carboplatin?

3. Should we skip chemo and try something else and hope for the best?

Any help is much appreciated as I am scared and worried.

I know you must be so worried! I wish I could help you with some of the technical aspects of your questions but I'm sure Tazziedog or Wookie can chime in there. But here are a couple of interviews that might ease your worries:

Lots of folks have ups and downs with chemo and most dog end up going through all of their sessions with flying colors. Often those sessions are not on schedule, just because bodies are such a dynamic, constantly changing organism, you can't always predict the timing will be perfect for chemo. I give a lot of kudos to your vet for being so cautious and holding off, that is the sign of a quality medical team.

I can't say for sure that the Rimadyl caused the issue but try not to worry about long-term effects unless you were seeing signs like black stools and GI upset. When I had a bad NSAID reaction (Previcox), I had a stomach ulcer but it cleared up when I stopped taking it and there were no long-term effects on my kidneys. 

Paws crossed that she's feeling better today. Remember, she went through major surgery, so ups and downs are definitely to be expected. Watch her closely and stay in contact with your vet, I'll bet she'll be just fine and you'll get right back on the chemo wagon real soon. Keep us posted.

It is possible for NSAIDS like Rimadyl to affect the kidneys although I usually see liver issues or GI upset with that drug.  Was a urine sample run before surgery?  If so was the urine concentrated then?  I assume she had IV fluids during and after her surgery but sometimes anesthesia can affect the body in unusual ways.

How old is your dog?  It is possible that she had borderline kidney function before surgery and the whole ordeal pushed her over the edge.  Has she had a Leptospirosis vaccine?  This is the time of year that we can see that disease and it does cause kidney failure.  I know that this can be prevalent in certain parts of the country such as Florida.

A creatinine of 2.5 is actually not that bad (normal at most labs is up to 1.7 to 2.0).  I would ask your vet how high her BUN and phosphorus were as well as her red cell count and urine protein levels.

Your oncologist is smart to delay treatment for now until you know more about her kidney status.

I'm sorry you've hit a rough patch. I don't know anything about the Adrimycin as we didn't use that, and we didn't have to deal with kidney issues. We did 6 rounds of carbo and Abby tolerated it well, except for her appetite being off for a day or so after treatments (not completely gone, but she just wanted bland food), and she got quite tired after the 5th and 6th treatments. Other than that she did fine. She enjoyed going for her appointments (the onc's office treated her like a rock star) so that was no worry either.

Still, having said that, I don't know if I'd do chemo again, or just try going totally holistic. It's such a tough call and hard to know what to do. I say that just because if you do end up deciding to not proceed with chemo, I don't want you to think it's the end of the world. There are other things you can do. We've seen all sorts of things here - some pups have great results with chemo, some don't. Some dogs have great results without chemo, some don't. It's a crazy, confusing disease.
