Chemo Nausea: Reglan (metoclopramide) make anyone else's dog crazy????

Charlie had his first round of chemo (carboplatin) thursday and has not had an easy time of it.  crying He whimpered most of Thursday night and most of the day yesterday.  No vomiting or diarrhea (just loose) but definitely nauseous and we have had a hard time getting his meds down with the peanut butter route he typically loves.  He's been lethargic and just lays there and stares.  He will eat after having Reglan 10 mg (also gets 40mg of Pepcid every 12 hours) but it seems to make him crazy.  He definitely no longer lays there after he has it, starts panting, can't lay down, gets wild-eyed and anxious.  When he is off of the Reglan he sleeps, but has a minimal appetite (eats a bit but not his typical appetite:  I have fed him both chicken and rice and boiled turkey burger and rice, I was going to go out and get some goat's milk today).

Should I ask for a different anti-emetic?  They won't be back in til Monday and I don't know whether to keep him on the Reglan or not, since he does eat a bit off of it and can at least rest and have more of his relaxed personality.  Anything else help with tummy troubles associated with chemo I can do for him?

Hey Mary Beth!

Couple of quick questions.....Any swelling where the IV was given? What pain meds is Charlie currently on?

Did they do any extra poking and prodding that could have made him be whining?

Was this an Oncologist who gave the Carbo?

Generally, certainly not always, a dog may experience some nausea or a bit of lethargy, but the wild eyed look and anxiousness is not something that is "usually" associated with the chemo. I'm certainly no Vet and it may, indeed, be a side effect that we just haven't seen nere.

I'm more familiar with Cerenia being used here as opppsed to Reglan. I did look up side effects on Dr Google and hyperactivity OR sedation could be a side effect. Just because it's in Google doesn't make it true, but just throwing it out there!

I'll be watching for a reply when you can.

We used Cerenia too, so I don't know Reglan.  Otis did sleep most of the day of chemo, so the lethargy might be normal.

Otis - 106 pound lab/Dane mix, lost his right front leg to osteosarcoma on Febuary 9, 2016.  Four rounds of carboplatin completed in April, 2016.  Lung mets August 25, 2016.  Said goodbye too soon on September 4, 2016.   Lost his adopted sister, Tess, suddenly on October 9, 2016. likely due to hemangiosarcoma.  

Wherever they are, they are together.

I may have missed the follow up to how Charlie was doing after the surgery to top his horrible pain and twitching. Was all that taken care of well before the chemo dose?

I have just caught up on your very involved and difficult journey so far with Charlie, I am so sorry you are going through so much adversity with your boy...... 

First off, I agree with what Sally is asking as far as Charlie's pain being managed before starting chemo..... My own sweet angel Lab girl Tripawd Polly had extreme difficulty with her doxorubicin chemo treatments, and my vets had her on both reglan AND Cerenia at the same time through her entire 6 treatments of the doxy, and she never experienced any kind of adverse effects from the anti-nausea meds, pepcid, metronidazole for diarrhea, mirtazapine appetitite stimulant, or antibiotics, and she was on all these at the same time. I am leaning toward him still being uncomfortable from pain, or even phantom pain , although my Polly never experienced that, so I am not as familiar with it. 

Thinking of you and him, and hoping he is feeling much better very soon, and that you can take a deep breath and relax a bit from everything you both have been through..... Sending {{{hugs}}}} your way 💜

Hi Mary Beth and Charlie heart

Eurydice did 6 sessions of Carboplatin with zero side effects, she took 1 pill of Cerenia the morning of chemo and 1 pill a day for 3 days after.

The reaction you are describing isn't the norm, most dogs here had zero or minor reactions to carboplatin.

It could be this Reglan pill versus Cerenia, I am no vet but I would call them to make sure the treatment is ok.

This is only his first session so Charlie's oncologist will be able to adjust the dosage or even postpone it if need be.

Eurydice 77kg/170lb Great Dane limping end of April 2016, amputation (right front leg/osteosarcoma) 4 May 2016 6 courses of carboplatin followed by metronomic therapy, lung mets found 30 Nov 2016. 3 courses of doxorubicin, PET scan 26 Jan 2017 showed more mets so stopped chemo. Holistic route April 2017. Lung X-ray 5 May 2017 showed several tennis ball size mets, started cortisone and diuretics. Miss Cow earned her XXL silver wings 12 June 2017, 13 months and 1 week after amputation and 6 1/2 months after lung mets, she was the goofiest dawg ever and is now happily flying from cloud to cloud woof woofing away :-)

Hi everyone, 

Thank you for all the replies, Charlie feels much loved! He saw this surgeon at Friendship on Wed morning and he was doing much better from his nerve pain.  Also, his swelling was way down.  He did have some new bruising at the surgical site but he was able to take the staples out which made Charlie much more comfortable.  We are continuing on the Ciproflaxin (2nd antibiotic added) another week since it seemed to be doing the trick with the seroma .  Surgeon has us watching for any drainage or opening and said his whole pain episodes could even have been an infectious process, we just don't know for sure.  He still gets Gabapentin every 8 hours and Amantidine once a day.  Stopped the codeine, doesn't need it anymore.

So the morning of Chemo was his best morning, running out the back door and wagging his tail.  During chemo he had to be sedated so was a panting mess from that.  And when that wore off he definitely had the 10-20% reaction to Carboplatin (of course) with no appetite, nausea, and vocalizing for a good 24 plus  hours.  The reglan definitely helped with the side effects but like I said he started to get crazy from it, anxious and wild-eyed, was almost worse than the nausea and no appetite.  I knew when this dog wouldn't eat chicken or turkey burger he felt terrible. Even when he had his horrible nerve pain he still ate.  No peanut butter either, our old stand by for administering pills.  So I kept him on the reglan for the first 2 days, but stopped it yesterday morning when I could still get him to eat enough to not be concerned.  He has been much better since.  No Reglan side effects so he can rest and he is now almost over the Carboplatin side effects.

I did get goat milk too, really helped.  Its the oddest thing.  Still won't touch canned dog food but will eat his rice and meat with a little goats milk or chicken broth.  He's happy now and yesterday was his best day yet.  First time we left him in almost 4 weeks. His sister Annie took care of him while we were out for a few hours.  I am definitely asking the Oncologist to switch him to Cerenia, maybe give him an injection at chemo, and possibly lower the Carboplatin if needed.  He got 150mg.

So he woke this morning after 10 hours since his last Gabapentin, a record!  And still not showing signs of needing it, so I think he is FINALLY turning the corner.  Hopefully we won't get the residual chemo effect now that is a more common reaction. 

It's nice to actually be able to share good news.

Thanks for sharing Mary Beth. I am over the moon to hear that things are looking up for Charlie! Dog knows you deserve it after all you've been through! I love the fact his tastes have changed, and why not, he loves his Mommas cooking! Would you go back to dog food? Nice one, Charlie!

To near Charlie is doing so much better is such great news!!! 🙂 You guys have been through an k incredibly rpugh time and we applaud you for staying the course! Yoj stayed strong and determined a dd finally got some answers!

Certainly not an Onco, but I woukd make sure Charlie is really over these horrible side effects he endured. Chemo is, I think, cumulative, so even with a reduced dose (and I certainly hope it is reduced!), he still may be susceptible to bad side effects. Also, I know the time in between chemo is increased sometimes. So that's another safety precaution you might consider.

And Charlie has to be sedated for each chemo treatment?? Poor guy! Some have decided to discontinue treatment should it continue to be too rough. The side effects subside and the pups get on with living life to the fullest! A Mastiff, Big Lou had to stop after two ttreatments because it compromised his quality too much. He just celebrated his ten or eleven month ampuversary and is happy as a clam!

I'm just throwing this out there, not in any way, shape or form to sound negative!! He will probably sail through his other treatments and this will all be behind you!!

THE most important thing right now is Charlie is feeling so good AND resting without pain! He really is an amazingly strong pup! You are powerful advocates for him!!

Thank you so much Sally.  I have definitely been thinking the same thing and will see how Chemo #2 goes and re-evaluate.  I am kind of dreading it now, but maybe with some adjustments he will have a bit of an easier time.  Hoping he doesn't have the typical rebound effect 4-5 days post chemo, or the low WBC 7-10 days in either.  That will all weigh in as well.  The balance between longevity and quality of life is so tough.  

I'm keeping notes until the 10th day or so and then will call in and see what they suggest.  I've become a bit wary of the Oncologist's recommendations after what happened when we went in for the consult.  I will see what she says and go with my gut on that one.

For now I am just enjoying him feeling better!

Little Biscuit took Reglan one time when he was having motility issues (caused by eating part of a pair of underwear blush).  He was ok with it the first day but after that he started acting really weird.  I was literally up all night long because he would not settle and was trying to climb the front room blinds and the walls.  It was like he was trying to get away from something.  He also acted like he was just not there.  I couldn't get him to look at me or respond to me.  I would try to hold him and he would try to claw his way up me.  It was very upsetting for me and Biscuit.  To say the least I did not give him another dose and told the vet what he had done.

Thankfully he finally passed the piece of underwear about 2 months later and hasn't had a problem since.

Restlessness is a common side effect of Reglan.  As well as trouble sleeping.  I would think that if he's having trouble with it, it's not the best medication for him.  I haven't tried Cerenia, but it sounds like it works well.  It looks like it's similar to Dramamine for people.

Thanks so much Donna and Marla for sharing the negative effects Reglan can have - Charlie has definitely gone downhill again Day 4-5 post chemo. Nauseous and not interested in too much food, he clearly doesn't feel well. Not eating the peanut butter again and will only eat a small portion of chicken and rice, and nothing else can be in it. He is back to just wanting to lay around and lethargic. I know this is a normal response but if I have a different anti-emetic I feel like I can get him more comfortable at least.
Hoping we end up with a majority of the 3 weeks having him feel good or at some point it just doesn't make sense to keep him going feeling lousy. He isn't in awful pain like he was before, but a nauseous, weak and exhausted life isn't a great way to spend the rest of his shortened life. 🙁
