The Fentanyl patch was applied the same day as surgery... not sure exactly when, but Athena had a big rectangle shaved on her hip with the patch stuck on it. It really made a difference in helping manage her pain those first few days, in my opinion.

Maggie came home with a fentanyl patch and also had both tramadol and an anti-inflam after her amp and she did fine.  My quad pug Obie had knee surgery in February and came home with the same mix.

My understanding of the patch is that it is applied just before or as the pup is weaned off of the IV pain killers.  In Obie's case he had IV fentanyl for surgery and post op while in the hospital, then fentanly patch when he came home. Both pugs definitely needed the tramadol in addition to the fentanyl.

I think the reason Dr. Pam would start the gaba ahead of time is because it works on the nerves.  Seems like a fentanyl patch in addition to IV pain killers would be overkill, but I could be wrong.  You could ask in the 'Ask a Vet' forum.
