On October 27, 2016, nearly 6 months after amputation, and 18 months since his cancer likely started, we lost Pofi to a recurrence of Soft Tissue Sarcoma in his spine quite suddenly.  His canine sister also succumbed to cancer on March 1, 2019 - we lavished her with our love in the interim, but life was never quite the same without her only real canine friend. Cliff kitty had to leave us, too, suddenly, in August 2019. Lucia kitty grieved all these losses, but helped us welcome two new Lurchers into our home and our lives, Shae and Barley.

Okay folks, bear with me, this will be a long post. Upfront: Oliver seems pretty great.

He will start chemotherapy on Thursday. The follow up tests showed that the has a genetic cell mutation (a c-kit PCR mutation assay was done and they found this mutation in locale 8. Typically, the mutation is found in locale 11, so this was also an anomaly). They have graded it a 2 out of 3, because cancerous cells were found in the lymph node that was next to the tumor in his leg, indicating early metastasis. The oncologist says that it could be entirely local, but obviously, since they cannot biopsy his other lymph nodes, there is no way of knowing for sure.

The good part of the bad news is that because it is a genetic mutation, they can do targeted chemotherapy. He will be on Palldia 3 times a week for 6 months. This drug is FDA approved and specifically targets the cell mutation. Our oncologist is also putting him on a course of prednisone as a conservative therapy to zap any cancer cells that could potentially be lingering. He will be on 20 mg. for the first 2 weeks and then taper down to 10 mg. for another two weeks. He will go in for blood work every 2 weeks for the first month, and then once a month for the following 5 months of treatment. Afterward, they will examine his internal lymph nodes using an ultrasound and determine whether he needs to continue the treatment, stop the treatment, or they need to add another drug.

His tumor was subcutaneous, too. So they know less about how these tumors act than they know about cutaneous ones. However, I read online that the prognosis is usually better for subcutaneous (i.e. less like to recur).

Also, I remember someone mentioned immune problems with chemotherapy. Our oncologist said that this drug and the targeted nature of it has not been associated with any immune issues (great news), but we will know well before we go on our honeymoon whether that's the case.

At least it doesn't sound like a worse case scenario anymore for Ollie for a change! Obviously the Onco will go over any potential side effects and how to stay a step ahead if there are any.

It's so wonderful to know Ollie is back on track and feeling soooooo much better! 🙂 So happy!

Some pretty good news there overall.  

Palladia has shown to be particularly effective for MST chemo for the reasons you outlined.  Also, though perhaps less impressively, has been effective for fighting STS (which is Pofi's cancer).  Seems to create a sort of 'stasis' per our oncologist (who we see again tomorrow).  Pofi has been on the Palladia 3 x per week protocol for one month.  He has tolerated it well - but I have also been giving him a Pepcid AC about 1/2 hour before giving him the Palladia out of an abundance of caution.

We are getting our first post Palladia bloodwork tomorrow.

Good luck to you and Pofi tomorrow--I hope it goes amazingly! That's great news about the Palladia. We have been giving Ollie 2 regular pepcid every 12 hours since he started all of these pills and 1 prilosec a day (as per our doc's recommendations) , so it sounds like it'll be good to continue something to avoid any sort of stomach issue.

He RAN up my friend's steps on Sunday, physically he seems great. He tries to do more than he should, so we have been more vigilant! On Saturday night, I went to dinner with a friend (Barrett was out of town). I had puppy in his inflatable cone. Came home, took him out, and we got back into the apartment and he ran over and started playing with something. It was a piece of bloody gauze. He had torn a hole into his tie on bandage and pulled out all of the bloody gauze! Like it was a toy with stuffing inside! Needless to say, he got to stay in the hospital that night so they could sedate him and change his bandage. But at least he was feeling better! They told me the wound looked much better though! We should be able to go until Thursday when when he gets his stitches out.

He is such a little trooper!! I will post a video tomorrow of him playing with a toy for the first time this past weekend. Little rascal still gets super pumped to get to the vet hospital. One of the techs even came up to him and he was so excited to see her. She said she slept in his cage after his second surgery because he couldn't get comfortable lol. They're like his extended family now... Just like you all!

The Onco recheck went really well - not sure I could hope for better.  They think he is doing fantastically.  The senior DVM student who brought him to the back for blood draw commented: "Boy, losing a leg hasn't slowed him down at all."

Exam revealed nothing they find worrisome. No concerns about lymph nodes.  Abdominal palpation was "unremarkable".  Nothing concerning with heart or lung sounds.  Lymph nodes not palpably swollen.  Blood work very normal - in fact one liver value we were watching that was slightly elevated has returned to normal.  He has gained two pounds.  Urinalysis showed +1 protein, not alarming, but something to watch while he is on Metronomic chemo.

They did give us Cerenia to give prior to pilling with Palladia as I think he has been less enthusiastic about food in the mornings - we will see if that helps. But it is also really hot and humid here and we don't have central air.  

Twice this week, when he seemed not so keen on breakfast he has led me upstairs to the master bedroom which is cool thanks to a portable AC and then he eats for me.  Smart boy.

When I said he did not seem to want to go on leash walks lately, but had spent 50 minutes romping at dog park on Saturday, the doctor said, "Well, it's really, really hot and humid!"

They did take a picture of the irritation on his nose - seems to come from licking his chops and is either irritated by chemo in his saliva or he is just more prone to hair loss there because of chemo and the licking is removing hair.
