After months of reading other people's experiences with their beloved hounds, you get a sense that while there are often common experiences, it can also be quite a unique journey. I thought I had some idea about what might happen as we got towards the end, but the last 24 hours have left me completely stunned.
A brief recap - Peda is 10+ year old shar-pei / akita, diagnosed with osteosarcoma in late June 2012. Front left leg amputated shortly after (actually it is her 9 month ampuversary in 12 hours!) A single largish lung met was discovered about 4 weeks ago. Palladia metronomic protocol commenced which has been generally tolerated (with a slight reduction in dosage). Overall, generally pretty reasonable month.
Last 5 days she has been more 'tired'. She has had really weeping eyes (but not infected or anything and not seeming to bother her - just lots of 'sleep' in the eyes). Still eating ok etc and happy to bounce around when heading out for a 'car trip' (walking is not so easy as also now has moderate / significant arthritis). Yesterday (Tuesday) she had review at vet hospital and CBCs were ok (neutrophils up from previous weeks and ok for palladia to continue - but still down on normal healthy range). She had slightly (really slightly) swollen left hock. No obvious pain reactions to usual poke and prod etc.
But by last night (Tuesday night - about 8 hours after vet trip) she was going rapidly down hill. She was breathing fast and struggling to get around, tripping over. She was almost snorting when she breathed rather than usual mouth panting. I gave her some tramal (in addition to her usual 300mg gabapentin) and she eventually settled. By morning, it was a repeat of the night before but struggling even more to get around. I had to lift her outside.
After lots of emails to the wonderful local vet and also her wonderful treating oncologist (who was on a day off!) we decided to get some better pain relief onboard. So added methadone to the mix and she settled in to a good sleep for most of day. She was quite sleepy and a bit wobbly but by mid-afternoon (6 hours later) she was becoming a lot more alert and more mobile.
This morning I really was worried that we were dealing with bone mets and that it was going to be a matter of a day or so if that. The wonderful local vet called in to see her on way home and to have a bit of a look at what was going on.
See - the weird thing is that her front leg really swelled up during the day, as well as her muzzle and lips. She is also running a fever. But she was a lot brighter - and starving! So lots of special food for dinner and she settled down quite happily.
But about hour or so later, there was again the rapid decline and we were back to last night / this morning level of distress. I gave her another injection but she took a long time to settle - I'm now parked on the couch next to her for the night (its 11pm here).
So - earlier this evening when the vet came the thought was maybe she has an infection of some sort. The vet had a good poke and prod and again wasn't really seeing any signs of bone met spread - certainly no real pain response to poking and prodding. Injection of antibiotics given, and also a non-steroidal to help with swelling. The other thing it might be - which would be really unusual - is the fever that Shar-pei's get (FSF). Except she's a mixed breed and as far as I know has never had an episode in her life (although she may have had milder episodes of being off that were never identified).
So at this stage of the day I am totally shattered. I've gone from really thinking this morning that the end was close, to now wondering what on earth is going on. I was feeling much better about things around 6pm, but not so good now. She's clearly unwell - but the question is what is going on.
We are off to the vet hospital tomorrow morning so hopefully we will be able to sort things out. While I know that the end is coming, it just seemed to be so completely out of the blue that my head spun. Now I'm not so sure that this is what is happening.
If anyone else has experienced this sort of thing, I'd be really grateful for some input. The temp makes me think infection, but not really sure what sort. Also the extreme pain seems unusual but I guess it depends what is infected. She is urinating normally (drinking a reasonable amount - it's been hot today).

Oh my...you have had a crazy 24 hours!!! I sure hope you get some answers soon. The only thoughts I have, which came to mind when you mentioned the swelling around her muzzle and lips...maybe an allergic reaction??? Bug bite??? I have owned two dogs now that were allergic to wasps and bees and it is that time of year when bugs come start coming out. Just a thought.

Wow, this has just gotta make your head spin, I'm so sorry you're both going through these ups and downs.
I know that when it comes to cancer, it's never a smooth ride, there are always bad days and good days. I have no idea what could be wrong with Peda but I do know that many times it's not as bad as pawrents think it is. The cancer just tends to magnify every wheeze and sneeze and make people very scared, but oftentimes these are just symptoms of another condition that has nothing to do with the cancer, and a vet visit often handles the issue. I really hope this is the case with Peda.
We are thinking of you both and have our paws crossed for better days ahead. Please let us know what the vet says OK?

Kristy and Peda...Try and old onto your sanity just a little longer and you'll have everything sorted out AND the meds will have a chance to do their job. Probably something unrelated to the cancer at all! Could be a Palladia reaction.....could definitely be the sharpei fever thing because she is, indeed, part sharpei!
My Bernie had sudden leg swelling ad muzzle swelling one night and it turned out to be an autoimmune thing that was treatable!!
We are all here with you----:-) distance does not brak the bond and connection of everyone on this site walking by your side!! Peda knows you are with her and knows our are doing everything with love.....and I know she is enjoying the heck out of being spoiled and loved like you do so well!!!
You are sooo strong and brave and so is Peda!!! You are not alone and we all. Certainly understand the fear and concern and we are here for you!!

Kristy and Peda, oh how I hope you get some answers soon! The fear of the unknown is horrible! I, like you fear the worse when Belle does not act herself, it is such a scary thing. It sounds like you have a great team of vets to help you find the answers! Belle and I are sending you pawsitive thoughts!

If you're seeing a pattern of improvement, however mild, followed by a down-swing, take note of when these events occur in relation to administration of drugs (or even foods) Peda may be receiving. My Tenny Klootchman (hemangiosarcoma) took us on an awful rollercoaster ride for a spell until we identified a pattern in the ups and downs. It turns out she had a bad reaction to Cerenia--which we were injecting in her to fight what was then a chronic nausea.

Oh geez! That IS quite a 24 hour ride. I look forward to hearing the vet results tomorrow. I, too, thought allergic reaction when you mentioned swelling. Could she have been bitten or stung in that back leg and that was the root of her hock inflammation and now mouth area, too? Yikes! Sending lots of HUGS your way! Try to remain calm and keep her calm, too. I know you are!

ACL tear in right hind leg 12/5/12 and scheduled ACL repair surgery 12/21/12. Pre-op xrays revealed osteosarcoma. Amputation 12/28/12. Chemo (carboplatin) started Jan 10, 2013 and ended on April 5, for a total of 5 doses. He handled carbo like a champ! No side effects. We started metronomic therapy at his third chemo and have been also doing some holistic treatments. He's a lively, playful 10 year old huskie-boarder collie and a very proud member of the Winter Warriors! Our love. Our funny little guy!

I just wanted to stop by and update you before I go and crash out for the evening. It's been a long day!
Peda was marginally improved this morning but we still had an unsettled night. At the vet she was fairly quiet and just lay down to let the vet examine her. Normally she is much more engaged and often pants. None of that - just tired!
After many blood tests and some other checks, have ruled out heart and kidney problems. Also most of her blood work was really good - slight increase in two liver enzymes but that can be for many reasons.
So after excluding many things, we are left with the assumption that she has vasculitis - either because of the Palladia or cancer. Neither is a particularly good scenario as if it is drug induced that's the end of treatment, and if its cancer related then its not great either.
However I am pleased to say that she is a very different dog this evening - has been quite bright and engaged and hopped in to the kitchen for dinner which was first time in a week! Some of the swelling is clearly going down and we actually haven't started treating the vasculitis yet.
I think next few days could be a bit touch and go ... but I'm happy to have her home and see her resting comfortably.

First things first . . . hoppy ampuversary Peda!!! I hope you get lots of yummy treats, I know your people will spoil you rotten.
Well, Kristy, I'm sorry that there weren't any clear answers from the vet. We all want to know what's exactly wrong and when we don't it can be so frustrating. Yeah, either of these scenarios aren't good, I'm sorry you didn't walk away with a more positive one. But as you've already seen, Peda doesn't care what the doc says, she feels good at the moment and that's all that matters.
This cancer thing is such a lesson in taking things day by day. It's harder for people, but you're getting there. It's easy to let you imagination run wild, but without knowing an exact answer, all anyone can do is take things as they come and be glad for the good times.

Even if she can't take Palladia anymore at least she is feeling better and her sparkle is coming back! There are all sorts of alternative things you can look into as an optional plan.
