Hello, I'm an American but have been living in India for 4 years now.i have been helping all the street dogs I can.well I found a puppy that on April 1st got hit by a vehicle and had to have his whole right back leg removed up to the hip.my wife and I have been fostering him while he recovers and we can find him a good home.he is almost 6 months old I believe.

My question is... the wound is healed and he has no problems getting around and doesn't seem to be bothered by anything.but he gets swelling areas like a balloon that has fluid in it.it usually opens and shrinks back down and becomes a scab.the vet said it's from him jumping and playing, which we have our dog that does play with him but it's impossible to separate them and calm them down being in the city.is there anything I can do to stop the swelling from happening? there's not much of a stub and it's impossible to wrap or cover since he is a male and his genitals get in the way. I'm new to amputation and I want to be able to take care of the dog so he isn't constantly having swollen areas that turn into open sores.any information anyone can offer would be greatly appreciated.thank you

Wow it is so cool what you are doing! THANK YOU to you and your wife for helping dogs, they are so blessed to have you.

So the dog had the amputation three months ago, and it sounds like he is still getting a seroma , possibly. Yes, the vet is right, seromas can be caused by too much activity too soon after surgery, but generally they are not a recurring thing and they tend to happen more in front than rear leg amputees. At least that's what we've seen here with members over the years.

What is the fluid like and where exactly is it accumulating? At the surgical site? Is it pinkish? Does it smell or look cloudy/infected at all? Does it bother him? And at the drainage site, does the sore heal afterward or are you having trouble with it?

Seromas can happen even a month after surgery and in a few cases, longer. I'm hoping that's what this is, because they usually resolve themselves after draining on their own once or twice.

I'm not a vet, but I'm thinking there's a chance it could be something like a suture reaction. Is there any chance you can get another vet to look at the area when it's recently drained? That way you can ask to have it cultured for MRSA or another type of infection.

To stop the swelling, there's really not much you can do at this point, as you've noticed. So I think the best thing is to get a culture done on the fluid to make sure it's not an incision infection.

Can only ditto everything  Jerry said, so we'll wait for your response.   It does  seem like a second opinion  and a culture  may be in order.

I haven't really gotten to see the fluid,it usually opens when I'm asleep, probably from him licking on it.the area it is happening is near the fold of where his hip or leg would meet his belly.or the bend in his hip or inner thigh.it usually starts by the area starting to swell up like a balloon and if you push on it you can tell that it's fluid,it will start turning dark reddish purple like a bruise.then as some of the fluid leaves the skin loosens up and gets alittle saggy and not too long after that is when it will open.the wound heals with no problems other than him licking it.and no it doesn't happen directly where he had his suture.that whole area is completely healed and haven't had no problems with it so far.i figured it was probably from him playing too much and too hard.

As for taking him to another vet,I probably could but the sad thing is most of the vets here only care about money so they are quick to push medicines and stuff without really telling you anything.the vet I go to is pretty good, especially since he only charges me half price for street dogs because I bring so many in.

Just to give you an example of how bad things are, when the puppy got hit really early in the morning and a nearby guy took him to an emergency animal hospital they wrapped his leg and maybe gave him some shots but told the guy the puppy needed to see a vet later.at which the guy contacted me after I woke up to tell me what happened.i went and got the puppy and took him to the vet. I'm no doctor but when they unwrapped it I knew he was losing his leg.the whole leg was split all the way open the whole length of the leg and it was almost black.the animal hospital had to of known it needed amputated,I guess they just didn't want to deal with it.ourvet even stayed after closing to do the operation plus not charging us the full amount.

Sorry this is turning into a book,I just thought I'd post here to try and get some answers.which you helped alot thank you.do you think if I created something or got something kind of like a compression wrap, would that help that area not move as much and cause irritation?

I figured in time that whole area will toughen up more and it not have issues.i am just trying to get him woundless since he's dealt with alot.

And I really appreciate you and the other person who posted thanking me for helping the dogs here.dogs have always been a big part of my life.and to move here and there just be so many dogs in need and most people just don't care.besides the dogs having to worry about food,cars and other dogs, they also have to worry about people abusing and torturing them, even killing them out of fun.

I don't have much money but do whatever I can.even if I don't have food for the dogs,I still go out every day and visit all the dogs to hang out with them and give them love.also check them for ticks or wounds.i just treat them with the respect they deserve.people here don't like Indian or street dogs, they want popular foreign breeds like labs and German shepherds.so the chances of this puppy getting adopted is slim, so he is a part of our family now.

Thank you for your kind words.once people realize that helping dogs and other creatures we are helping a part of ourselves.the fact that dogs give me love and attention and accept me,where most humans don't is more than enough for me to do my best for them.

Maybe one day when I figure out how to post pictures here I can share pictures of some of my dog friends here.

Oh my gosh, thank you for taking time to explain everything. It is not a book, really, no apologies necessary.

First, you are truly a generous, loving, and enlightened soul. THANK YOU for helping this pup and all the others. I know the situation for India's dogs is not good, and you are a remarkable individual for doing what you are able to do. Talk about making a difference in the world. I cringed and got teary-eyed when I read about how you found this pup, and I cannot imagine how many others there aren't lucky enough to find someone like you. One of my dreams is to visit India but I've been warned by a traveling vet (see below) that it can be extremely heartbreaking for animal lovers.

Question: Where in India are you? The vet I mentioned is highly regarded pain specialist vet here in America who is connected with some progressive-minded Indian veterinarians. Together they are trying to improve conditions for dogs and other small animals. If you want to PM me your location I can see if this vet is anywhere near you.

Meanwhile, that is an interesting situation with the fluid drainage. Yes, sometimes a hematoma can form when a dog is often falling down on the amputation side. Does he hit that site when he is playing hard? These fluid pockets are small and come and go, and usually don't cause long-term damage. So I think that might be what your vet was trying to describe. Oh and I understand why you are working with that vet, so you work with what you've got and it's perfectly understandable. If the dog isn't losing energy or appetite he's probably OK and doesn't have an infection, but it would still be good to see if you can get the vet to test the fluid just in case. I know that's a tough thing to do though. 

I honestly don't know if a compression wrap will help. That is such a tough area to cover. If he is falling quite often and hitting that area, what will help him not fall are core-strengthening exercises to help him stay upright and have better balance. Young dogs who lose a leg often struggle with balance and coordination as puppies, and that could be the cause of the fluid build-up. I'm not a vet, so take that for what it's worth (this is based on what rehab vets tell us). But if you have the ability to help him get stronger with exercises that strengthen his back and core muscles, you might see this problem go away. Our Tripawds Gear blog posts about core-strengthening and exercise have lots of ideas, and while much of the equipment we mention is for sale, there are many ways to replicate them with household objects.
